What a change order needs to survive a dispute
Written direction, named and dated
Record who asked for the change, when, and how. A name and a date turn 'they told us to' into evidence.
Scope described in the contract's own language
Mirror the wording of the original scope so it is obvious what is additional and what was already included.
Costs broken into line items
Labor hours, material quantities and equipment listed separately. A single lump sum invites a negotiation you will lose.
Overhead and profit stated, not buried

Is this the AIA G701 form?
No. AIA G701 is a copyrighted document published by the American Institute of Architects and must be licensed from them. This is an independent change order form that captures the same commercial information: scope, cost breakdown, schedule impact and signatures. If your contract specifically requires G701, buy it from AIA.
Do I have to give my email to download it?

What markup should I put on a change order?
Check your contract first — most specify an allowable overhead and profit percentage for change orders, and that stated cap is the number that governs. If the contract is silent, use your normal rate and state it plainly on its own line.
What happens if the GC will not sign?
Do not start the work. If you are directed to proceed anyway, send written notice that you are proceeding under protest, keep daily records of labor and materials, and photograph everything. That paper trail is what a time and materials claim is built on.
